Effect of acute experimental aortic stenosis on coronary circulation.

چکیده
• The effect of aortic stenosis on the coronary circulation warrants further investigation because the few previous studies led to conflicting conclusions based on small series of experiments. Moreover, aortic stenosis presents a unique situation for a study of the determinants of coronary blood flow. On the one hand, the location of the stenosis, between the left ventricle and the aortic origin of the coronary arteries, creates a systolic pressure gradient unfavorable for coronary flow. This flow-reducing factor is augmented by the increase in extravascular compression resulting from the prolonged systolic ejection phase in aortic stenosis. Indeed, a decrease in coronary blood flow was noted by Green when he produced aortic stenosis by inflating a balloon in the aortic orifice. On the other hand, it has generally been observed that coronary blood flow increases whenever the load on the left ventricle is increased. It would seem logical that a similar increase in load would result from constriction of the outflow tract, whether it be proximal or distal to the coronary ostia. It has been repeatedly demonstrated that aortic constriction distal to the coronary ostia results in an increase in coronary blood flow. Moreover, Gregg and Shipley noted an increase in coronary blood flow in a modified preparation of aortic constriction that was analogous to aortic stenosis. In their preparation, the ascending aorta was constricted,

Two-dimensional echocardiography in experimental coronary stenosis. II. Relationship between systolic wall thinning and regional myocardial perfusion in severe coronary stenosis.
To determine if there is a quantitative relationship between systolic contraction abnormalities (demonstrated by two-dimensional echocardiography) and reduced myocardial perfusion in a setting of moderate and severe coronary stenosis, we created 70% or 90% reduction in circumflex coronary artery diameter in open-chest dogs. Transient ischemia was induced by superimposing increased myocardial ox...
متن کاملPercutaneous valved stent implantation above the coronary ostia.
BACKGROUND In recent years, some experimental and clinical studies on transcatheter aortic valve implantation (TAVI) have been conducted. TAVI is indicated in patients with calcified pure or predominant aortic stenosis. The risk of this technique is still high. Aortic valved stent implantation above the coronary ostia might avoid blocking the coronary ostia. METHODS AND RESULTS Twenty healthy...

متن کاملOxygen consumption in subepicardial and subendocardial regions of the canine left ventricle. The effect of experimental acute valvular aortic stenosis.
Moderate and severe levels of experimental valvular aortic stenosis (VAS) were produced in anesthetized, open-chest dogs to determine the effects of VAS on subepicardial (EPI) and subendocardial (ENDO) blood flow, O2 extraction, and O2 consumption (MVO2). Regional flow was determined by microspheres, and O2 saturation and extraction were analyzed by a three-wavelength absorbance microspectropho...
متن کاملCoronary artery disease and aortic stenosis in the transcatheter aortic valve replacement era: old questions, new paradigms: the evolving role of percutaneous coronary intervention in the treatment of patients with aortic stenosis.
Significant coronary artery disease (CAD) is present in up to 50% of patients with symptomatic aortic stenosis (AS). Although numerous databases have shown that the addition of bypass grafting to an aortic valve replacement (AVR) nearly doubles the mortality, surgical series have shown that leaving significant coronary stenosis untreated increases AVR mortality.1–4 Thus, the standard of care fo...
